Welcome to our digital packet pick up for the 2021-2022 school year!
Click each form to open and view. Then, please use the download button to download each form to your computer. DO NOT fill out in your internet browser (safari, internet explorer, chrome, etc.), it will not give you the access you need. Use Adobe Acrobat DC or Preview on a Mac to complete these forms. You should use the submit button to submit these forms via your email platform tomhroffice@mhrschool.com. Please contact Mrs. Schultz if you have questions.
Family Pledge of Safety Measures
Provided by the Archdiocese of Chicago, this Family Safety Pledge is Mandatory to sign and return to MHR prior to the start of the school year. We must have this important safety pledge on file from your family before your child returns to school on Wednesday, August 25th.
Please Submit and Return
These forms are required for all families and must be submitted and returned digitally.
Download them and fill completely. At the end, please click the submit button and your email platform will send a completed form to Mrs. Schultz.
Family Information Card / Photo-Academic Work Permission / Family Handbook Ackowledgement
These three forms are required for ALL families and should be downloaded and submitted back to Mrs. Schultz. Please read through and fill out carefully. Do not fill out in your internet browser, please download to your computer or device.
Acceptable Use Policy
The Acceptable Use Policy is required for ALL students in K-8.
Packet Pick Up has gone digital, but we’d still love to see your faces, Raiders! Send in photos of the excitement in your house for us to share! Email publications@mhrschool.com or connect with us on our social platforms to share a picture today.
Family / Grade Specific
These forms are only needed for some families. If you need to complete them, please do so digitally (or by hand when applicable) and then use the submit button at the end to send to Mrs. Schultz directly. Forms needing to be filled out by hand must be submitted on your child’s first day of school.
Movie Permission Slip
This form needs to be completed for families with students in grades 5-8.
Medication Dispensed During School
This form needs to be completed for any student requiring medication dispensed during school. Portions of this can be completed digitally and then printed, but the form must be signed by a physician. Please turn in a hard copy.
Ethnicity Data Collection
This form needs to be submitted for ALL new students, including preschool.
Gmail code of conduct
The Archdiocese has requested that all students from grades K-8 be assigned a school Gmail account. Please fill this form out for an incoming Kindergarten student only.
Health Forms
Health forms are to be completed by hand only. Many doctors offices and clinics provide them, however we are posting them here for your convenience.
Students in Kindergarten need to submit vision forms.
Students in Kindergarten, 2nd and 6th grade need to submit dental forms.
Students new to MHR, and in grades K and 6 need to submit child health exam forms.

This is for families interested in purchasing Assignment Notebooks (Grades 2-8), Sketchbooks (Grades 4-8), and MHR Pencil Cases
Lunch and Milk
This is for those ordering snack milk and families interested in the cost of our hot lunch program for the 2021-2022 school year.
