Dear Parents,

To help ensure the safety of our students, the State of Illinois has required that all students participating in interscholastic sports have a sports physical on file starting with the 2019-2020 school year. The Archdiocese of Chicago Office of Catholic Schools is complying with this state policy.

For students in Kindergarten or 6th grade, a physical with a report of vaccinations is already expected. This will suffice as long as the box on the bottom of the form is circled YES (next to Interscholastic Sports) and signed by your doctor.   

Physicals will be dated and valid for 395 days, obviously more than a school year and more than a calendar year. Hopefully, this will carry students through the summer months.  

We are asking everyone to complete a physical as soon as possible, especially those students hoping to play fall sports. This year we are setting an August 1st deadline. You can bring the form to the school and if we are not around, you can drop it off at the rectory. Make sure it is in a sealed envelope marked: SCHOOL PHYSICAL.  

Since this is considered confidential information and the school is responsible for the compliance of this policy, school personnel will be the only eyes to see and file these forms. We will be recording the dates of the physical; however, you will need a new physical each year. For this year, students playing fall sports will be benched if the physical form is not returned by August 1st. After this year, students will be benched if they do not have an updated physical before their sport begins.  

Please be patient as we work through this new process. We appreciate everyone’s cooperation and look forward to another successful athletic year!
